NES Membership

Terms & Conditions

1) Scope & Purpose

NES™ Memberships provide access to member-only pricing and wellness perks across thyroid, hormone, metabolic, and weight-care services. Memberships are not health insurance and do not cover or guarantee medical services, diagnoses, prescriptions, treatments, or outcomes.

Care is delivered by NES™ clinicians pursuant to medical judgment and applicable law. Services, procedures, and products are billed separately unless expressly listed as a membership perk.

2) Billing & Renewal

Memberships are recurring monthly subscriptions billed automatically to your saved payment method. Your billing date is the date you enroll and repeats on the same date each month.

If a charge fails, NES™ may retry payment; if unsuccessful, membership benefits may be suspended until payment information is updated and processed.

3) Active Status & Benefit Eligibility

Membership benefits apply only while your membership is active. If you cancel or your payment fails, perks end at the close of your current paid cycle.

Membership fees do not include physician visits, labs, procedures, imaging, prescriptions, or supplements unless expressly listed as a perk (e.g., “5% off consults”).

4) Visits & Member Rates

Visits and benefits are determined by the membership tier selected at enrollment. All consultations at NES™ are billed on a pay-per-visit basis unless specifically listed as a perk. Active members receive tier-based benefits as outlined below:

• Discounts apply to eligible NES™ services and in-office supplements.
• “Nutrient shots” refer exclusively to vitamin shots.
• All included injections are administered in-office and do not roll over month-to-month.
• Member discounts cannot be combined with other promotions, coupons, packages, or insurance benefits.
• Benefit availability and pricing may change with 30 days’ notice.

5) Nutrient Shots & In-Office Services

• Memberships may include 1 or more nutrient shots per month, depending on tier.
• Included shots refer only to B12 or B-Complex injections.
• Excluded services: Glutathione injections, hormone injections, and IV therapies are not included.
• Any unused monthly shot does not carry over to the next month.
• All injections are administered in-office only during clinic hours.

6) Supplements & Discounts

Supplement discounts apply only to in-office purchases at NES™, not to online purchases from the NES Shop or third-party websites.

7) Pricing & Benefit Changes

NES™ may update membership pricing, tiers, or included benefits with 30 days’ advance notice (via email and/or website update). Your continued membership after the effective date constitutes acceptance of changes.

8) Insurance, HSA/FSA, & Eligibility

NES™ does not bill insurance and is considered out-of-network. Superbills can be provided upon request. Some services or products may be eligible for HSA/FSA reimbursement depending on your plan. You are responsible for confirming eligibility with your administrator.

9) Labs, Testing, Prescriptions & Medical Judgment

Ordering of labs, advanced testing, or prescriptions (including thyroid, hormone, or weight-management therapies) is at the discretion of your NES™ clinician based on medical necessity and safety.

Lab and test fees are billed separately by NES™ or third-party vendors unless specifically noted. Free blood draws are not included in any membership tier.

10) Scheduling & Priority Access

Members may receive priority scheduling, depending on their tier. Priority does not guarantee same-day or next-day appointments. Missed appointment and late-cancellation policies still apply.

11) Cancellations, Pauses & Refunds

Memberships can be cancelled anytime via your NES™ account or by written request. Cancellations take effect at the end of the current billing cycle.

Membership fees are non-refundable once billed, and NES™ does not prorate partial months. Members may request one pause per 12 months; benefits are inactive during a pause.

12) Communications & Consent

By enrolling, you consent to receive operational emails, texts, or portal messages related to your membership and care. You may opt out of marketing messages separately.

13) Age & Residency

Members must be 18 years or older (or have a parent/guardian for pediatric care, if offered). Certain services may be Arizona-only or limited by clinician licensing and telehealth rules.

14) Emergencies & Scope of Practice

NES™ does not provide emergency or urgent care. In the event of an emergency, call 911 or visit the nearest emergency department. NES™ focuses on endocrinology-related and integrative care; referrals may be made as clinically appropriate.

15) Termination

NES™ may terminate a membership for non-payment, misuse of benefits, or inappropriate conduct with written notice. Access remains active through the end of the current paid cycle unless termination is immediate for cause.

17) Governing Law

These terms are governed by the laws of the State of Arizona, without regard to conflict-of-law principles.
